What you choose to provide

The quiz asks for your ZIP code, audience category, date of birth, marital status, basic work or service information, existing insurance, desired coverage, who or what you want to protect, whether you have had a heart attack, stroke or cancer in the last three years, and your name, email and phone. Your date of birth and health answer are saved only if you agree and submit. You may leave without submitting. Do not enter medical records, Social Security numbers, a street address or another person’s details.

When information is saved

Before submission, answers stay in the page’s temporary memory. When you review the collection statement and select “Agree & save my summary,” Arken LifeGen stores your answers, contact details, any campaign identifiers present on your visit, the collection permission and time, an internal review status, tier, notes and follow-up date if added by our team. We use this information to organize your coverage inquiry, prioritize internal follow-up from Tier 1–5 using non-health answers, and, with your submission permission, contact you about this inquiry by a manually placed call or email. Tier is an internal inquiry-priority label, not an insurance eligibility or underwriting decision. The health answer does not produce an eligibility decision or increase or decrease your internal lead-quality score. Format checks do not verify your identity or eligibility.

Sharing and contact

Inquiries are stored using our website hosting and database providers. With your permission, Arken LifeGen may share your inquiry and contact details with a licensed insurance professional for follow-up about life-insurance coverage. Arken LifeGen and that professional may contact you by a manually placed call or email. Automated calls, marketing texts and unrelated email campaigns are not authorized. You can withdraw follow-up permission at support@arkenlifegen.com or by deleting your inquiry. Earlier inquiries authorized saving only and do not gain referral permission retroactively.

Your choices and deletion

You may leave before submitting or view your summary without saving. After submission, save your summary and private deletion receipt. Use that receipt on the Privacy choices page to delete the matching inquiry from our application database. Keep it private: anyone holding it can delete that inquiry. Records remain stored until deleted; deleting your inquiry does not delete copies you downloaded or hosting-provider backups and technical logs.

Technical information

The hosting provider may process IP addresses, browser details and access logs to operate and secure the site. We do not request device geolocation. We use Meta Pixel for advertising measurement and analytics, including a PageView event when pages load. Meta may receive information such as your IP address, browser and device details, page URL and other standard event data. Meta may use this information for measurement, reporting, ad delivery and personalization under its own policies. You can manage cookies and similar technologies through your browser settings and review Meta’s privacy choices and settings. Campaign parameters in the page address may be saved with an inquiry.
